2 Detailed Scope Of The Task: 1) The Subject Of The Contract Is The Provision Of Transport And Pick-Up Services For Students With Disabilities From Their Residential Address In The Commune. Lubsza To Educational Institutions In Brzeg And Kup.2) The Total Number Of Children Transported And Transported Is 13 (Including 9 Children To Brzeg And 4 Children To Kup), It Should Be Taken Into Account That The Number Of Students May Change During The Service Period.3) Children Transported And Transported Must Be Cared For By A Guardian During Journeys, At Least 1 Guardian Per One Vehicle. For Safety Reasons, The Driver Cannot Be A Guardian.4) Children Will Be Picked Up From Specific Residential Addresses In The Lubsza Commune5) Children Will Be Transported To Indicated Educational Institutions In The Towns Of Brzeg And Kup6) Transport Will Be Carried Out By Means Of A Fleet Of Vehicles On School Days In The Period From January 1, 2026 To June 30, 2026 In Accordance With The School Year Calendar Approved By The Minister Of National Education (Taking Into Account Work On Saturdays Off).7) If There Is A Need To Transport A Preschool Child, The Transport Will Be Carried Out In Accordance With The Organization Of The Kindergartens Work, Also During Breaks And Holiday Breaks As Part Of The Kindergartens Work (Except For The Summer Break).8) To Calculate The Price Of The Offer, The Estimated Total Number Of Kilometers To Be Covered On All Routes During The Order Implementation Period (Assuming ~110 Days Of Schooling, Per Day) Established By The Ordering Party Preparation Of Specifications, There Are No Kindergarten Children To Be Transported), I.E. 21,010 Km. 9) The Number Of Kilometers Given Above May Be Subject To Ad Hoc Adjustments Resulting From Changes In The Lesson Plan Or Student Absences And Will Be Agreed On An Ongoing Basis By The Contractor. 10) For The Purposes Of Calculating The Offer Price In The Offer Form, The Estimated Total Number Of Kilometers To Be Traveled On All Routes During The Order Execution Period, Determined By The Ordering Party, Should Be Used. However, The Settlement Of The Service With The Contractor Will Take Place For Actual School Days And Actually Performed Transport On The Routes Indicated In Annex 1 To The Contract And Taking Into Account The Maximum Daily Mileage Limit Indicated There.11) The Number Of Kilometers May Change Due To Changes In The Lesson Plan, Student Absences And Will Be Agreed On An Ongoing Basis By The Contractor.12) The Given Number Of Children Is Current As Of The Date Of Preparation Of The Description Of The Subject Of The Contract And Is Expected To Change During The School Year13) The Contractor Will Have To Adapt The Number Of Vehicles Used For Transport To The Actual Needs So That The Child Transport Service Is Performed Properly And On Time.14) A Detailed Description And Method Of Order Execution Is Included In The Description Of The Subject Of The Order (Opz), Constituting Annex No. 1 To The Swz.
