The Subject Of The Order Is A Transport Service - Transport, In 2026, Of Elderly People With Physical Disabilities (Delivery And Pickup), Who Are Residents Of The Day Care Home Day Care Home Senior+ In Czechowice-Dziedzice, Hereinafter Also Referred To As Ddp In The Documents. The Term Transport Should Be Understood As The Transport Of Seniors From Their Place Of Residence To The Ddp, While The Term Removal Should Be Understood As The Transport Of Seniors From The Ddp To Their Place Of Residence After The End Of The Seniors Stay At The Ddp On A Given Day.2. The Provision Of Specialized Passenger Transport Services Covers 13 Participants Of The Senior+ Day Care Home, Including 1 Participant In A Wheelchair, Together With A Guardian Provided By The Ordering Party (Including A Guardian Of 14 People) - From The Place Of Residence Of Individual Participants To Ddp And Back, From Monday To Friday. The Service Is Provided As A Social Assistance Task.3. Transport Will Be Provided Twice A Day: 1) Morning Course - Participants Will Be Picked Up From Their Places Of Residence And Transported To Ddp; 2) Afternoon (Return) Course - Pickup Of Participants From Ddp And Transport To Their Places Of Residence. 4. Basic (Estimated) Parameters Of The Transport Service: 1) Average Daily Route: 40 Km 2) Service Provided Only On Working Days (Monday To Friday, Excluding Saturdays, Sundays, Holidays And Public Holidays)3) In 2026 The Number Of Working Days Will Be 251 Days (Days Off, I.E. Weekends + Holidays: 114)4) Estimated Number Of Kilometers To Be Covered As Part Of The Service: 10 040 Km (Approximate Information).5. Requirements For The Vehicle Used To Provide The Elderly Peoples Transport Service. The Contractor Must Provide At Least One Vehicle That Meets The Following Requirements:5.1. Equipment And Adaptation1) Min. 13 Seats + 1 Seat For The Caregiver (Seated);2) Ramp Or Platform/Elevator For A Wheelchair (With A Valid Certificate);3) Places For Securing The Wheelchair With Immobilizing Belts;4) Air Conditioning And Heating;5) Seat Belts For All Seats; 6) Interior Lighting;7) Markings And Equipment In Accordance With Currently Applicable Law.5.2. Technical Condition1) The Vehicle Is Technically Efficient, Approved For Traffic;2) Clean And Well-Kept;3) Has A Valid Technical Inspection And Third-Party Liability Insurance. Comments To Point 5: 1) The Production Date Of The Vehicle Which Will Provide The Passenger Transport Service Cannot Be Lower Than 2012. 2) In The Event Of A Breakdown Of The Vehicle Used To Provide The Passenger Transport Service (The So-Called Basic Vehicle), The Contractor Is Obliged To Ensure That The Passenger Transport Service Is Provided With A Replacement Vehicle That Meets All The Minimum Requirements Specified In This Swz.6. Requirements For The Driver: 1) Driving License Of The Appropriate Category; 2) At Least 1 Year Of Experience In Transporting Elderly Or Disabled People; 3) Ability To Help Elderly People With Mobility Disabil
