Tenders are invited for Supply, Delivery, Installation, Commissioning and Maintenance of Slide Warmer 02 Non Refundable Fee (Rs.) 2000 Original Receipt obtained from the Shroff of the District General Hospital. Nawalapitiya upon payment of a non-refundable bid fee mentioned above along with a certified copy of the Business Registration Certificate should be submitted to obtain bid forms. Bid documents may be obtained from the Accounts Division of the District General Hospital, Nawalapitiya between 9.30 a.m. and 3.00 p.m. on normal working days from 26.09.2025 until 16.10.2025. If a representative is calling over to collect the documents, a letter of authorization from the applicant and identity card for the identification purposes should be submitted. Bid documents may be inspected free of charge at the Accounts Division of the District General Hospital, Nawalapitiya before obtaining the bid documents. Separate bid securities should be submitted for each package as mentioned in the bid document in favour of Director, District General Hospital, Nawalapitiya. Bid security should be attached to the original bid and submitted. Bids should be submitted as per the conditions and specifications mentioned in the bid documents. A pre bid meeting for the above items will be held at 11.00 a.m. on 09.10.2025. Bids will be opened immediately after closing of bids at 11.30 a.m. on 17.10.2025 at the Office of the Director, District General Hospital, Nawalapitiya. Applicant or an authorized representative may be present at the time of opening of bids. If a representative is representing, a letter of authorization and the identity card for the identification purposes should be submitted.
