I. Subject Matter Of The Order: The Subject Of The Order Is The Replacement Of Elevator Cranes As Part Of The Task: Improvement Of The Technical Condition Of The Hospitals Utility Facilities By Replacing Elevator Cranes In The Buildings Of The Provincial Hospital In Tarnobrzeg Ii. Description Of The Subject Of The Order: 1. Cpv Code According To The Common Procurement Vocabulary: 51500000-7 Machinery And Equipment Installation Services 42416100-6 Elevators 2. The Subject Of The Order Includes: 1) Supply Of Elevator Cranes; 2) Installation Of Elevator Cranes; 3) Dismantling The Replaced Cranes, Cabins And Installations; 4) Disposal Of Dismantled Devices In Accordance With Applicable Regulations; 5) Obtaining All Decisions Necessary And Required By The Construction Law Or Other Legal Provisions (In Particular Permits), Arrangements, Consents, Notifications, Opinions, Etc. For The Implementation Of The Subject Of The Contract; 6) Preparation And Delivery Of Technical Documentation With The Requirements Specified By The Office Of Technical Inspection, Certification In Accordance With Applicable Standards, And Approval Of The Lift For Use By The Office Of Technical Inspection; 7) Participation Of The Contractor In The Activities Of Approval For Use By The Office Of Technical Inspection; 8) Ensuring Periodic Inspections Required By The Provisions Of The Office Of Technical Inspection During The Warranty Period. 3. A Detailed Description Of The Subject Of The Order Is Included In Annex No. 2 To The Swz - Description Of The Subject Of The Contract And Annex No. 3 To The Swz - Price Form. 4. The Subject Of The Contract Will Be Made Of Materials Provided By The Contractor. The Ordering Party Reserves The Right To Assess The Equivalence Of The Proposed Solutions, As Well As The Right To Use Expert Opinions In This Respect. In Case Of Discrepancies, The Ordering Party Reserves The Right To Choose The Construction And Assembly Materials To Be Used. 5. All Materials And Labor Should Comply With The Requirements Of The Documentation, Applicable Standards And Meet The Requirements Of Certificates And Regulations Allowing For Use In Health Care Facilities. At Each Request Of The Ordering Party, The Contractor Will Provide The Ordering Party With A Technical Data Sheet For The Material Or Device As Well As Any Certificates And Declarations Confirming Compliance With The Applicable Standard Or Technical Approval. 6. All Work Must Be Performed In A Manner That Does Not Violate The Terms Of The Manufacturers Warranty For The Built-In Materials And Installed Equipment And Accessories. Upon Receipt Of The Subject Of The Order, Confirmed By The Protocol Referred To In § 9 Section 6, All Rights Under The Manufacturers Warranty Are Transferred To The Ordering Party. If The Ordering Party Raises Any Objections As To The Impact Of The Work Performed On The Above-Mentioned Warranty, The Contractor Is Obliged To Present A Written Statement From The Manufacturer That The Method Of Performing The Work Does Not Violate The Manufacturers Warranty. 7. During The Warranty Period, The Contractor Should, At Least Once A Year, Carry Out An Annual Service Inspection In Accordance With The Provisions In The Executive Documentation. 8.
