The Subject Of The Order Is A Service Called: Winter Cleaning Of Sidewalks, Parking Lots, Squares And Transport Stops And Emptying Trash Bins In 2024. The Contractor Will Be Obliged To Maintain Sidewalks, Squares, Parking Lots And Transport Stops During The Winter And Empty Trash Bins On The Basis Of The Contract Concluded With The Ordering Party. The Work Should Be Performed On All Working Days, Weekends And Holidays. The Scope Of The Service Provided Will Include:1. Cleaning Sidewalks, Squares, Parking Lots And Transport Stops In The Following Way: 1) Removing Mud, Snow, Ice And All Other Dirt (Paper And Leaflets, Foil And Aluminum Packaging, Sand, Plant Remains: Leaves, Flowers, Branches Of Trees And Shrubs, Birds And Dog Feces, Cigarette Butts, Bottles And Cans Of Alcoholic Beverages) And Eliminating Slipperiness In The Assigned Area; The Assigned Area Should Be Kept Clean At All Times, 2) Slipperiness And Black Ice Should Be Eliminated Up To The Surface Hardened With Paving Slabs, Concrete Blocks Or Asphalt; The Removal Of Slipperiness In The Commissioned Area Should Be Done Using A 30% Sand-Salt Mixture, 3) Snow And Ice Should Be Removed To The Edges Of Pedestrian Routes Or Squares And Parking Lots, Creating Dumps While Maintaining A Minimum Of 80% Of The Width Of The Cleaned Surface; There Should Be Gaps In The Piled Snow On Sidewalks At The Level Of Pedestrian Crossings; It Is Unacceptable To Remove Snow From The Cleaned Area Onto The Adjacent Road, 4) Any Dirt Collected From The Contracted Area Should Be Taken To A Landfill, 5) No Grass Should Grow On The Contracted Areas, Especially Near Sidewalk Edges And Street Curbs, 6) The Contractor Shall Secure The Equipment And Sand And Salt On Its Own,7) The Contractor Is Fully Responsible For Accidents Occurring On The Commissioned Premises Resulting From Improper Maintenance, In Particular When The Cause Of The Incident Is Unremoved Black Ice Or Slipperiness.2. Emptying Street, Park And Bus Stop Bins In The Following Manner: 1) All Bins Should Be Emptied On An Ongoing Basis And Overfilling Is Unacceptable, 2) All Pollutants Within A Radius Of 5 Meters From The Bin Should Be Collected, 3) Pollutants Selected From The Bins And Around Them Should Be Taken To A Landfill,4) In Winter, Baskets And The Area Within A 1M Radius Around Each Of Them Should Be Cleared Of Snow In A Way That Allows Them To Be Used,5) Baskets That Have Been Overturned Or Moved To Another Place Should Be Placed In Their Current Position ,6) The Ordering Party Will Provide Bags For Dog Packs And Bags For Garbage Bins In Places Where Metal Inserts Are Vandalized Or Their Service Is Impossible For Other Reasons.3. Detailed Requirements Of The Ordering Party: 1) The Ordering Party May Exclude From The Monthly List The Areas Of Sidewalks, Squares, Parking Lots Or Bus Stops Where Construction Works Are Being Carried Out Or Where It Is Impossible To Perform The Assigned Task For Other Reasons. 2) List Of Sidewalks, Parking Lots And City Squares To Be Cleaned In The Winter Season Of 2024, Constitutes Annex No. 1 To The Swz (Area: 7,130 M2). List Of Street And Park Bins To Be Emptied In The Winter Season In 2024. Constitutes Appendix No. 2 To The Swz (222 Pieces, Including 3 Dog Packages). The List Of Transport Stops And The Baskets Placed Therein Within The Administrative Boundaries Of The City Of Bielsk Podlaski To Be Maintained In The Winter Season In 2024 Is Attached As Annex No. 3 To The Swz (900 M2 And 33 Baskets). The Total Area To Be Maintained From Both Of The Above. Annexes Is 8030 M2. The Ordering Party May Change The Size Of The Cleaned Area Or The Number Of Baskets Depending On Their Needs One Week In Advance. Bags And Keys For Dog Packages Will Be Provided By The Ordering Party.3) In The Offer Form (Appendix No. 4 To The Swz), The Bidder Should Specify Net Unit Prices: A) For Cleaning Up Mud, Snow, Ice And Other Dirt And Eliminating Slipperiness From 1 M2 Of The Square, Parking Lot, Sidewalk And Traffic Stop Within A Period Of 1 Month, Including Sprinkling The Ordered Surface With Sand And Salt With A Concentration Of 30% And Transporting Pollutants To The Landfill, B) For Monthly Emptying Of One Bin And Pollutants Within A Radius Of 5 Meters From It, Including Transporting The Collected Pollutants To The Landfill, 4) The Coordinator Of The Work On The Part Of The Ordering Party Will Be: Mr. Wiesław Sukaczuk, Tel. 857318149 And Mr. Marcin Kuptel, Tel. 857318131, And The Contractor Will Appoint A Coordinator By Entering Personal Data And Telephone Number In The Contract. 5) The Fee For The Service Will Be Settled On A Monthly Basis Based On The Protocol Acceptance Of The Completed Works.6) The Contractor Concludes Insurance Contracts At His Own Expense And Bears Full Liability For Accidents Occurring On The Commissioned Sections Of Squares, Sidewalks And City Parking Lots And Transport Stops Resulting From Their Improper Maintenance. Employment Requirements Based On The Employment Relationship: The Ordering Party On Based On Art. 95 Of The Public Procurement Act, Specifies In The Description Of The Subject Of The Contract For Services The Requirements For The Employment By The Contractor Or Subcontractor On The Basis Of An Employment Relationship Of Persons Performing Activities Related To The Implementation Of The Contract Indicated By The Ordering Party, If The Performance Of These Activities Involves Performing Work In The Manner Specified In Art. 22 § 1 Of The Act Of June 26, 1974 - Labor Code (Journal Of Laws Of 2023, Item 1465).1. The Ordering Party Requires That, When Implementing The Order, The Contractor Employs, Under An Employment Contract Within The Meaning Of The Provisions Of The Act Of June 26, 1974 - Labor Code (Journal Of Laws Of 2023, Item 1465), Persons Who Will Perform Winter Maintenance Of Sidewalks , Parking Lots, Squares And Transport Stops As Well As Emptying Bins Covered By The Order.2. The Contractor Is Obliged To Document The Employment Of The Persons Referred To In Point 1. During The Execution Of The Order, At Each Request Of The Ordering Party And Within The Time Specified By The Ordering Party In The Request, The Contractor Will Submit To The Ordering Party: A) A Declaration Of The Contractor Or Subcontractor On The Employment Under An Employment Contract Of Persons Performing The Activities Referred To In Point 1. This Declaration Should Include In Particular: Precise Identification Of The Entity Submitting The Declaration, Date Of Submission Of The Declaration, Indication That The Activities Covered By The Request Are Performed By Persons Employed Under An Employment Contract, Indicating The Name And Surname Of The Employed Employee, The Date Of Concluding The Employment Contract. Work, The Type Of Employment Contract, The Scope Of The Employees Duties And The Signature Of The Person Authorized To Submit A Declaration On Behalf Of The Contractor Or Subcontractor, Or B) Copies Of Employment Contracts Certified By The Contractor Or Subcontractor, Respectively, Of Persons Performing The Activities Referred To Above During The Execution Of The Order. Declaration Of The Contractor Or Subcontractor. Copies Of Contracts Should Contain Information, Including Personal Data, Necessary To Verify Employment Under An Employment Contract, In Particular The Name And Surname Of The Employed Employee, The Date Of Conclusion Of The Employment Contract, The Type Of Employment Contract And The Scope Of The Employees Responsibilities, Or C) A Statement Of The Employed Employee Containing Name And Surname Of The Employed Employee, Date Of Conclusion Of The Employment Contract, Type Of Employment Contract, Scope Of The Employees Duties And The Employees Signature.3. For Failure By The Contractor Or Subcontractor To Employ Persons Performing The Activities Indicated In Point 1 Under The Employment Contract, The Ordering Party Provides For A Sanction In The Form Of The Contractors Obligation To Pay A Contractual Penalty In The Amount Specified In The Public Procurement Contract Template. Failure By The Contractor To Submit The Evidence Requested By The Ordering Party To Confirm That The Contractor Or Subcontractor Has Met The Employment Requirement Under The Employment Contract Within The Deadline Set By The Contracting Authority Will Be Treated As The Contractors Or Subcontractors Failure To Meet The Requirement To Employ Under The Employment Contract The Persons Performing The Persons Specified In Section 1 Activity.4. The Ordering Party Reserves The Right To Carry Out An Inspection To Verify Whether The Persons Performing The Activities Specified In Point 1 Are Employed Under An Employment Contract, In Particular: A) Requesting Declarations And Documents Confirming Compliance With The Above-Mentioned Requirements.
