1) The Subject Of The Order Includes The Renovation Of The Premises Of The Public Health Center In Żabno (Doctors Offices, Treatment Rooms, Nurses And Midwifes Offices, Registration, Waiting Rooms And Adaptation Of Toilets To The Needs Of Disabled People. The Implementation Of The Subject Of The Order Will Improve The Accessibility Of Medical Services To Patients, Including Elderly People And Disabled People (With Limited Mobility).2) The Task Is Planned To Carry Out Works Relating To: • Dismantling Works, • Tiling Works, • Painting Works, • Filling Walls, • Installing Suspended Ceilings, • Laying Pvc Floor Coverings, • Interior Joinery, • Installing Sanitary Fittings, • Covering Stairs With Tiles, • Electrical Installation, • Ventilation Installation, • Water And Sewage Installation, • Central Heating Installation. 3) Detailed Description And Scope Of The Subject Of The Order Are Determined By: Design Documentation - Annex No. 5 To The Swz Bill Of Quantities - Annex No. 4 To The Swz, Stwiorb - Annex No. 6 To The Swz Designed Provisions Of The Contract - Annex No. 7 To The Swz4) Works Included In The Object Of The Contract Include Works Concerning Eligible Costs (Bill Of Quantities, Items 1 To 217) And Non-Eligible Costs (Bill Of Quantities, Items 218 To 280). Due To The Above, The Contractor Will Be Obliged To Issue Two Invoices For The Works Performed - One For The Value Of The Works Concerning Eligible Costs And The Other For Non-Eligible Costs. The Value Of Individual Scopes Of Work Will Be Determined In The Contractors Offer Form. 5) The Contractor Undertakes To Perform Construction Works And Hand Them Over To The Ordering Party In Accordance With The Contract Requirements Of The Construction Law And Other Applicable Regulations And Standards, As Well As In Accordance With Technical Knowledge, Construction Practice And Occupational Health And Safety Regulations. 6) The Ordering Party Expects The Contractor To Provide The Price For The Execution Of The Order As A Lump Sum Price, Which Includes The Execution Of All Elements Of The Order That Are The Subject Of The Drawing And Text Of The Order Documents, Including Vat Applicable On The Date Of Submission Of The Offer. 7) The Draft Contractual Provisions Adopt The Principle Of A Lump Sum Remuneration, So The Bill Of Quantities Included In The Order Documentation Are Also Intended To Enable The Valuation Of Individual Elements Of The Works.7) Before Signing The Contract With The Selected Contractor, The Ordering Party Requests That The Contractor Present A Detailed Bid Cost Estimate, Prepared On The Basis Of The Design Documentation, Stwiorb, Bill Of Quantities, And A Possible On-Site Visit Carried Out By The Contractor.8) Moreover, In Under The Subject Of The Order, The Contractor Is Obliged To: A) Ensure That The Works Are Managed By Persons With Qualifications In The Specialties Necessary For The Proper Performance Of The Entrusted Task And Keep An Investment Implementation Or Construc
