1) The Subject Of The Order Is The Performance Of Construction Works Consisting In The Reconstruction Of The Sidewalk, Exits And Parking Bays On The District Road No. 2766W Ul. 3 Maja In Józefów On The Section Up To The Exit To Plot No. 3/11 Round 42 To Plot No. 35 Rev. 36 - Section Of Approximately 210 Meters - The Scope Indicated In The Graphic Attachment. 2) The Task Involves Setting The Edges, Making A Sidewalk Surface Of Concrete Cubes, Making A Surface Of Exits From Concrete Cubes, Making A Surface Of Yellow Tiles With Protrusions And A Surface Of Concrete Cubes In Parking Bays For The Total Amount Of Pln 300,000.00, 3) Scope Of Work: A) Dismantle The Existing Surface Of The Sidewalk And Exits, Edges; B) Perform Earthworks By Making Troughs; C) Make A Base Of Crushed Aggregate 0-31.5 Mm - The Upper Layer, After Compaction, 15 Cm Thick For The Sidewalk And 20 Cm For The Exit, Where The Ratio Of The Secondary Module E2 To The First E1 Must Be Less Than Or Equal To 2.2, And E1 Must Be At Least 80 Mpa.;D) Place New Curbs And Concrete Edges On A Bench With C12/15 Cement Concrete Support;E) Lay The Pavement Surface With New Concrete Cubes Placed On A 1:4 Cement-Sand Ballast, Min. 5 Cm. Sidewalk Surfaces Should Be Laid Maintaining A Transverse Slope Of 2% Towards Green Areas, The Surface Must Be Silted With Sand, And Longitudinal Slopes Cannot Exceed 5%. The Sidewalk Should Be Made Of Rectangular Concrete Cubes 20X10 Cm Without Bevels, 6 Cm Thick, In Yellow And Gray Colors, Separated By A Strip Of Graphite Cubes (As In The Previous Sections). F) Lay The Surface Of The Exits/Parking Bays With New Concrete Cubes Laid On A Cement-Sand Ballast 1:4 Thick, Min. 5 Cm, Double T Cubes, 8 Cm Thick, Graphite/Gray In Color, The Surface Must Be Silted With Sand, G) The Curbs On The Side Of The Road Edge Are Not Subject To Replacement. 4) As Part Of The Investment, The Height Of All Infrastructure Elements, I.E. Valve Boxes, Gate Valves, Manholes And Hydrants Appearing In The Planned Sidewalk Surface - If Any - Should Be Adjusted. The Adjustment Should Be Carried Out In Consultation With The Network Operator. 5) In Places Where There Are Pedestrian Crossings, Make Two Rows Of Yellow Tiles With 6 Cm Thick Protrusions On A Cement-Sand Ballast. 6) The Works Must Be Carried Out In Compliance With The Relevant Regulations And Work Safety Instructions When Performing Construction Works; The Contractor Is Obliged To Develop, Approve And Implement Temporary Traffic Organization For The Duration Of Construction. After The Works Are Completed, The Contractor Will Restore The Permanent Traffic Organization. 7) Employees, Material And Equipment Necessary To Complete The Subject Of The Order Are Provided By The Contractor; 8) The Contractor Is Obliged To Manage Materials From Demolition On His Own. 9) After Completing The Works, The Contractor Will Prepare As-Built Documentation, Which Should Include, Among Others, Test Results, Certificates, Declarations Of Conformity, Pho
