1. The Subject Of The Order Is The Removal Of Solid Municipal Waste Collected In The Facilities Of The Silesian Rehabilitation And Spa Center. Dr. A. Szebesty In Rabka Zdrój At Ul. Dietla 5, 34-700 Rabka Zdrój, Consisting Of: A) Removal Of Solid Municipal Waste Collected In 1,100 Liter Containers Located On The Premises Of Śląskie Centrum Rehabilitacyjno-Uzdrowiskowe Sp. Z O. O. (Hereinafter Referred To As The Center), With A Fixed Frequency - Monday, Wednesday And Friday Of Each Week From 1000 To 1100, Settled According To The Rate Per 1 M3 Of Waste. B) Removal Of Outsized Waste (Such As Furniture) - From A Place Agreed With The Ordering Party, With A Frequency Depending On The Ordering Partys Needs, Settled According To The Rate Per 1 Ton Of Waste. C) Removal Of Leaves, Grass And Wood Waste From Places Agreed With The Ordering Party, With Frequency According To The Ordering Partys Needs, Settled According To The Rate Per 1 M3 Of Waste. D) Removal Of Plastic Waste (E.G. Pet Bottles, Foil) From Places Agreed With The Ordering Party, With Frequency According To The Ordering Partys Needs, Settled According To The Rate Per 1 M3 Of Waste. E) Waste Paper Removal, According To Needs, According To The Rate Per 1 M3.2. The Service Specification Is Included In Annex No. 1 To The Offer Offer Form And Annex No. 1.1. Pricing Form. The Remaining Terms Of The Contract Are Specified In Annex No. 3 To The Specification Of The Terms Of The Contract Designed Provisions Of The Contract.3.
