As Part Of The Task In Question, The Contractor Will Be Obliged To Deliver Brand New Teaching Aids For Sensory Integration, Hereinafter Referred To As Aids, To Selected Schools In The Wrocław Commune And Kobierzyce Commune (The Facilities And The Quantities To Be Delivered To Each Of Them Were Indicated In The Spreadsheet For Task 1), I.E.: 1) Beanbag Seat (9 Pieces) 2) Sensory Tunnel (2 Pieces) 3) Animation Tunnel (1 Piece)4) Weighted Plush Toy (3 Pieces)5) Set Of Gymnastic Equipment (2 Sets)6) Set Of Mats Stimulating Foot Muscles (4 Sets)7) Balancing Platform With A Maze (2 Pieces)8) Sensory Discs (1 Set)9) Set Of Rehabilitation Shapes For Children (1 Set)10) Set Of Balls With A Bag (1 Set)2.A Detailed Description Of The Subject Of The Order Is Included In Annex No. 1 To The Swz - Spreadsheet With Opz For Task 1 (Hereinafter Referred To As Opz 1).3.Note: The Ordering Party Informs That Some Of The Aids Constituting The Subject Of This Task (As Described In Ozp1) Are To Constitute A Medical Device And As Such: • Are To Have A Medical Device Certificate Issued In Accordance With The Act On Medical Devices Of May 20, 2010. On Medical Devices (Journal Of Laws Of 2017, Item 211, As Amended) Or Equivalent (The Ordering Party Will Recognize As Equivalent A Document Confirming The Compliance Of The Product With The Properties Declared By The Manufacturer Or Specified In The Regulations Or Standards. In The Absence Of Such A Certificate, The Ordering Party Requires A Declaration From The Contractor About The Lack Of A Certificate Requirement For The Given Equipment)• Is Subject To A Vat Rate Of 8%.4.The Contractor Is Obliged To Transport The Subject Of The Order, Unload It, Bring It In And Assemble It In The Indicated Rooms In A Given School (Delivery And Assembly Will Take Place Directly To The Given School Indicated In Opz1) And Clean Up Any Waste And Impurities Arising As A Result Of The Delivery And Remove Any Damage That May Have Occurred.5.All The Above-Mentioned Activities Will Be Performed At The School Premises.6.The Ordering Party Requires That The Delivered Aids Be Brand New, Unused And Undamaged7.The Aids Offered Must Include All Accessories And Additional Equipment Indicated In Opz1.8.The Parameters Of The Subject Of The Order Presented In Opz1 Constitute The Technical And Qualitative Minimum Expected By The Ordering Party.9.When Delivering The Aid, The Contractor Is Obliged To Attach Operating Instructions In Polish.10.The Subject Of The Order Must Be Packed In A Way That Protects It Against Damage. The Contractor Is Liable For Damage Or Loss Of The Subject Of The Contract Until It Is Handed Over To The Ordering Party At The Place Of Delivery, Confirmed By The Acceptance Protocol.11.Delivery Should Be Made Only On Working Days From 8.00 A.M. To 3.00 P.M., Excluding Saturdays - Or On Another Date Approved By The Ordering Party.12.The Contractor Is Obliged To Agree The Delivery Date (Day) Directly With The Ordering Party.13.Due To The Fac
