1. The Subject Of The Order Are Construction Works Consisting In The Renovation Of X-Ray Rooms In Building No. 10 Located On The Premises Of The Police Cynology Department In Sułkowice.2. The Full Scope Of Works Was Included In The Functional And Utility Program, Constituting Annex No. 1 To Swz.3. Symbol According To The Common Public Procurement Vocabulary (Cpv): 45000000-7 - Construction Works, 4531000093 - Electrical Installation Works.4. All Built-In Devices And Construction Products Should Have Appropriate Approvals Or Certificates.5. The Contractor Is Obliged To Complete The Order On The Terms And Conditions Described In The Essential Provisions Of The Agreement, Constituting Annex No. 9 To The Swz.6. The Ordering Party Recommends A Site Inspection Of The Facility. The Date Of The Inspection Should Be Agreed With A Representative Of The Investment And Renovation Department, Tel. 723 965 977 Or 47 725 58 43.7. The Ordering Party Does Not Allow The Submission Of Variant Offers.8. The Ordering Party Allows The Execution Of Part Of The Order To Be Entrusted To A Subcontractor.9. The Ordering Party Requests That The Contractor Indicate In The Offer The Parts Of The Order Whose Performance Will Be Entrusted To Subcontractors, And Provide The Names Of Any Subcontractors, If They Are Already Known, According To Annex No. 7 To Swz.10. The Ordering Party Requires Contractors To Demonstrate That They Have People Capable Of Performing The Order And Having The Necessary Knowledge And Experience. For This Purpose, The Contractor Must Submit A Declaration, The Template Of Which Is Attached As Annex No. 5 To The Swz, That The Persons Who Will Participate In The Execution Of The Order Have The Required Qualifications In Accordance With The Construction Law Act (Journal Of Laws Of 2025, Item 418, As Amended), Hereinafter Referred To As Construction Law, And Current Industry Qualifications, Including: Sep G1 Qualifications For Operation Of Up To 1 Kv In The Field Of Electrical Installations, Fully Enabling The Implementation Of The Subject Orders.11. The Ordering Party In Accordance With Art. 95 Of The Act Requires Contractors To Demonstrate That They Employ, On The Basis Of An Employment Relationship, At Least One Person Who Will Participate In The Implementation Of The Task, Holding A Sep G1 Qualification Certificate For Operation Up To 1 Kv. For This Purpose, The Contractor Attaches A Declaration, The Template Of Which Is Attached As Annex No. 6 To The Swz, That The Person Who Will Perform Work In The Field Of Electrical Installations Is Employed Under An Employment Contract, If The Performance Of These Activities Involves Performing Work In The Manner Specified In Art. 22 Section 1 Of The Act Of 26 June 1974 - Labor Code (Journal Of Laws Of 2025, Item 277, As Amended), Hereinafter Referred To As The Act - Labor Code. 12.
