Part Ithe Subject Of The Order Is The Provision Of Care Services Provided By: - 1 Caregiver In The Senior Club, Under Part I, For 25 People Requiring Support In Everyday Functioning Due To Age, Health Or Disability, In Accordance With The Following Detailed Conditions And Requirements: 1) Services Will Be Provided In The Form Of Day Care In The Senior Club By A Caregiver In The Period From 08/2026 To 07/2028 R. With The Possibility Of Extension For The Duration Of The Project.2) Care Services Will Be Provided In One Place Of Service, I.E. Senior Club, Ul. Stok 40, 26-341 Mniszków And Will Include Support For Participants During Their Stay In The Senior Club.3) The Total Scope Of The Order Includes The Provision Of Care Services In Accordance With The Current Needs Of The Participants And The Operating Schedule Of The Senior Club, Established By The Ordering Party, During The Duration Of The Order.4) The Caregiver Will Provide Services For An Average Of 42 Hours Per Month. An Hour Of Service Is A Clock Hour (60 Minutes), Counted As The Actual Time Spent Providing Care Services In The Senior Club. The Services Provided Do Not Include The Time Needed To Reach The Place Of Service Provision.5) Payment Of The Contractors Remuneration Will Be Based On The Actual Number Of Hours Of Providing Care Services, Confirmed On The Basis Of Verified Working Time Records.6) The Scope Of Care Services Provided At The Senior Club Includes In Particular: - Assistance In Meeting The Daily Life Needs Of Participants During Their Stay At The Senior Club, - Support In Hygiene Activities Appropriate To The Form Of Day Care, - Assistance In Care Activities Recommended By A Doctor, To The Extent That Does Not Require Medical Services, - Ensuring Care, Safety And Support In Social, Integration And Health-Promoting Activities Carried Out In The Senior Club, - Supporting Participants In Social Contacts And Active Participation In The Life Of The Senior Club,7) Care Services Are Provided In The Local Community, As Part Of The Activities Of The Senior Club, Respecting The Individual Needs And Capabilities Of Participants.8) The Contractor Is Not Entitled To Reimbursement Of Costs Incurred During The Provision Of The Service, E.G. Purchasing A Meal. The Costs Incurred By The Caregiver Are Not Reimbursed By The Ordering Party.9) The Contractor Is Obliged To Provide Care Services, Taking Into Account The Subjectivity Of The Participants, Including Respecting The Right To Respect And Protection Of Dignity, Intimacy, In Particular In The Case Of Activities Of A Hygienic And Personal Care Nature, As Well As The Sense Of Security And Protection Of Personal Rights.10) The Contractor Is Obliged To Ensure The Continuity And Appropriate Quality Of The Provision Of Care Services On The Days And Hours Of Operation Of The Senior Club, By Properly Organizing The Work Of Caregivers And Adapting The Scope Of Services To The Needs Participants.
