The Subject Of The Order Is The Renovation Of A Driveway For People With Disabilities Along With The Renovation Of The Main Entrance Of The Pro Familia Independent Public Health Care Center In Radomsko. The Scope Of Works Includes: 1) Demolition Works 2) Foundations 3) Construction Of A Reinforced Concrete Structure Of Stairs 4) Construction Of A Steel Structure Of A Ramp 5) Construction Of Partition Walls 6) Replacement Of Entrance Doors 7) Reconstruction Works Detailed Description Of The Subject The Order Constitutes The Design Documentation (Technical Design, Stwior, Bills Of Quantities) - Annex No. 7 To The Swz. In The Event Of Discrepancies Between The Provisions Of The Design Documentation And This Swz (In Particular The Draft Provisions Of The Contract), It Is Assumed That The Provisions Of The Swz Shall Prevail. Note: Due To The Adopted Nature Of The Contractors Remuneration (Lump Sum), The Bills Of Quantities Have Only An Informative Function And Do Not Constitute The Basis For Calculating The Price By The Contractor. The Ordering Party Requires Activities Related To The Execution Of Works In The Industry Construction Work Was Performed By Persons Employed By The Contractor And Subcontractor On The Basis Of An Employment Relationship Within The Meaning Of Art. 22 § 1 Of The Act Of June 26, 1974 - Labor Code (I.E.: Journal Of Laws Of 2025, Item 277, As Amended). Self-Employment). The Contractor, Subcontractors And Further Subcontractors Will Include Labor Costs In The Contracts Concluded In Connection With The Performance Of This Contract, The Value Of Which Cannot Be Lower Than The Minimum Remuneration For Work Determined Pursuant To Art. 2 Section 3-5 Of The Act Of October 10, 2002, Minimum Remuneration For Work (I.E.: Journal Of Laws Of 2024, Item 1773). Before Starting The Works, The Contractor Is Obliged To Mark The Construction Site (Information And Warning Boards) And Notify The Owners Of Neighboring Plots And Any Persons Staying On Neighboring Plots About The Works Being Carried Out. Supplementary Works Required By The Ordering Party - Execution As-Built Documentation In Polish (Bound In Binders With A Detailed Table Of Contents) Containing At Least: A) The Original/Photocopy Of The Construction Log, B) The Original Written Declaration Of The Construction Manager: • About The Execution Of The Works In Accordance With The Construction Design, The Final Decision Of The Building Permit And The Construction Law, • About Ensuring The Proper Condition And Order
