1. The Aim Of The Order Is To Improve The Functioning And Social And Professional Activation Of 100 People In The Crisis Of Homelessness Or At Risk Of Homelessness By Providing Access To Specialized Services.2. The Following Activities Will Be Carried Out As Part Of The Order: 1) Meetings With A Psychologist Whose Task Will Be To Understand The Participants Situation, Support In Difficult Situations And Help In Acquiring New Skills (Including Recognizing Emotions And Constructively Dealing With Difficult Emotions, Ways Of Dealing With Stress, Anxiety And Anger), Motivating To Take Action, Working On The Participants Sense Of Security, Supporting In The Adaptation Process. The Service Does Not Concern The Preparation Of Psychological Diagnoses, Opinions Or Judgments. The Service Will Be Provided On Average 40 Hours/Month. The Total Number Of Hours During The Order Execution Period Is: 720; 2) Meetings With A Lawyer Whose Task Will Be To Provide Legal Advice, Assistance In Writing Letters, Legal Assistance In The Field Of, Among Others: Family Law, Housing Law, Inheritance Law, Social Security Law, Consumer Bankruptcy. The Service Will Be Provided On Average 20 Hours/Month. The Total Number Of Hours During The Order Execution Period Is: 360; 3) Meetings With A Career Advisor Whose Task Will Be To Motivate You To Take Up Professional Activity, Help In Preparing Application Documents, Support In Contacts With Labor Office Advisors, And Use Of Available Instruments On The Labor Market. The Service Will Be Provided On Average 40 Hours/Month. The Total Number Of Hours During The Order Implementation Period Is: 720; 4) Meetings With An Addiction Therapist, Whose Task Will Be To Make A Preliminary Diagnosis Of The Severity Of The Addiction, Motivate For Change, And Provide Information About Places Where Help And Support Can Be Obtained. The Service Will Be Provided On Average 20 Hours/Month. The Total Number Of Hours During The Order Implementation Period Is: 360; 5) Meetings With A Health Educator Whose Task Will Be To Build Health Awareness In Order To Promote A Healthy Lifestyle, Provide Information About Treatment And Prevention Options, And Facilitate Access To Medical Services And Other Forms Of Support. The Service Will Be Provided On Average 10 Hours/Month. The Total Number Of Hours During The Order Execution Period Is: 180.3. A Detailed Description Of The Subject Of The Order Is Included In Annex No. 4 To Swz.4. Due To The Specific Nature Of The Order, The Ordering Party Reserves The Right To Not Complete The Subject Of The Order At The Level Of 20% Of The Gross Order Value.5. The Contractor Is Obliged To Cooperate With The Ordering Party At Every Stage Of Implementation Of The Subject Of The Contract.6. The Description Of The Subject Of The Order Is Supplemented By The Conditions For The Implementation Of The Subject Of The Order Contained In The Contract Template, Which Constitutes Annex No. 5 To The Swz.
