Software Supply Services – Zp/73/2026 - Implementation Of The Rcmc System. The Subject Of The Order Is The Implementation Service And Delivery Of The Regional Center For Digital Medicine System. The Contract Template Is Attached As Annex 4 To The Swz. Before Awarding The Contract, The Ordering Party Will Call On The Contractor Whose Offer Was Rated The Highest To Submit, Within A Specified Period, Not Less Than 10 Days From The Request, The Following Subjective Evidence, Valid As Of The Date Of Submission, Or A Certificate Replacing The Relevant Subjective Evidence Confirming The Fulfillment Of The Conditions For Participation In The Procedure And The Lack Of Grounds For Exclusion: 1) In Order To Confirm The Fulfillment Of The Conditions Regarding The Economic Or Financial Situation Specified In Section 5.3 Point 3) Of Swz, The Ordering Party Requests From The Contractor: A) A Document Confirming That The Contractor Is Insured Against Civil Liability In The Scope Of Activities Related To The Subject Of The Contract For The Guarantee Sum Specified By The Ordering Party In Point 5.3 Letter C); 2) In Order To Confirm The Fulfillment Of The Conditions Regarding Technical Or Professional Capacity Specified In Section 5.3 Point 4) Of The Swz, The Contracting Authority Requests From The Contractor: A) A List Of Services Performed, And In The Case Of Repeated Or Continuous Services, Also Performed, During The Last 3 Years, And If The Period Of Running The Business Is Shorter - During This Period, Along With Their Value, Subject Matter, Dates Of Performance And Entities For Which The Services Were Or Are Being Performed, And Attaching Evidence Specifying Whether These Services Have Been Performed Or Are Being Performed Properly, A List Of Persons Directed By The Contractor To Perform The Public Procurement, In Particular Responsible For The Provision Of Services, Quality Control Or Management Of Construction Works, Along With Information On Their Professional Qualifications, Qualifications, Experience And Education Necessary To Perform The Public Contract, As Well As The Scope Of Activities Performed By Them And Information On The Basis For The Disposal Of These Persons B) Certificates Of An Independent Entity Dealing With Certifying The Contractors Compliance With Specific Quality Management Standards, Including Accessibility For Disabled People, If The Ordering Party Refers To Quality Management Systems Based On The Relevant Series Of European Standards And Certified By Accredited Units. 3) In Order To Confirm The Lack Of Grounds For Excluding The Contractor From Participation In The Procedure, The Contracting Authority Requests The Following Subjective Means Of Evidence: A) Information From The National Criminal Register To The Extent Specified In Art. 108 Section 1 Point 1 And 2 Of The Public Procurement Law Issued No Earlier Than 6 Months Before Its Submission; B) Information From The National Criminal Register To The Extent Specified In Art. 108 Section 1 Point 4 Of The Public Procurement Act Regarding The Prohibition To Apply For Public Procurement As A Penal Measure - Issued No Earlier Than 6 Months Before Its Submission; C) A Copy Or Information From The National Court Register Or The Central Registry And Information
