The Subject Of The Order Is The Provision Of Collection, Transport And Disposal Services For Medical Waste And Non-Hazardous Waste For Międzyleski Specialist Hospital In Warsaw. 2. The Service Includes Collection, Transport And Disposal Of The Following Waste From Group 18 01 (In Accordance With The Regulation Of The Minister Of Climate Of January 2, 2020 On The Waste Catalogue). A) Code 18 01 01 - Surgical And Procedural Instruments And Their Remains (Except 18 01 03) - In The Amount Of 0.20 Mg Per Year, B) Code 18 01 02 - Body Parts And Organs And Containers For Blood And Preservatives Used For Its Storage (Except 18 01 03) - In The Amount Of 4.0 Mg Per Year, C) Code 18 01 03 - Other Waste That Contains Live Pathogenic Microorganisms Or Their Toxins And Other Forms Capable Of Transferring Genetic Material, Which Are Known Or There Are Reliable Grounds For Believing That They Cause Diseases In Humans And Animals (E.G. Infected Diapers, Sanitary Pads, Pads), Excluding 18 01 80 And 18 01 82 - In The Amount Of 250.00 Mg Per Year, D) Code 18 01 06 – Chemicals, Including Chemical Reagents, Containing Hazardous Substances – In The Amount Of 1.0 Mg Per Year, E) Code 18 01 08 – Cytotoxic And Cytostatic Drugs – In The Amount Of 0.10 Mg Per Year, F) Code 18 01 04 – Waste Other Than Those Mentioned In 18 01 03 (E.G. Fabric Or Plaster Dressings, Bedding, Disposable Clothes, Diapers) - In The Amount Of 0.10 Mg Per Year, G) Code 18 01 09 - Medicines Other Than Those Listed In 18 01 08 - In The Amount Of 0.020 Mg Per Year. 3. The Service Includes The Provision Of Adr Consulting In Accordance With Applicable Regulations. The Advisers Responsibilities Include: A) Supervising Compliance With The Requirements For The Transport Of Dangerous Goods In A Given Enterprise, B) Advising The Enterprise On The Transport Of Dangerous Goods And Preparing An Annual Report On The Enterprises Activities In The Field Of Transport Of Dangerous Goods, C) Monitoring Other Practices And Procedures Related To The Enterprises Operations 4. The Adr Advisor Should Have All Current And Required Documents/Certificates. The Contractor Is Obliged To:  Have Appropriate Authorizations To Transport, Store And Dispose Of Them Or Have A Valid Contract With A Company Dealing With Transport, Storage And Disposal During The Period Of The Order.  For The Collection Of Medical Waste, Which Will Take Place Every 72 Hours Between 700 To 1,200 From The Designated Medical Waste Warehouse At The Międzyleski Specialist Hospital In Warsaw.  To Collect Waste Using A Means Of Transport Adapted For This Purpose.  To Exercise Due Diligence In Fulfilling The Obligations Of The Contract.  To Perform The Service, Which Must Be Carried Out In Accordance With The Applicable Legal Provisions. - For The Disposal Of Medical Waste Originating From The Ordering Partys Facilities In A Disposal Plant With Spare Processing Capacity Located In The Voivodeship, In
