The Subject Of The Order Includes: 1) Advanced Als Phantom For An Adult - 1 Pc. 2) Advanced Pals Phantom For A Child - 1 Pc. 3) Advanced Pals Phantom For An Infant - 1 Pc. 4) Bls Phantom For An Adult With The Ability To Control The Resuscitation Quality, Set - 2 Pcs. 5) Bls Phantom For A Child With The Ability To Check The Quality Of Resuscitation - Phantom With A Set Of Accessories Plus A Control Tablet - 2 Pcs. 6) Infant Bls Phantom With The Ability To Control The Quality Of Resuscitation. Phantom With A Set Of Accessories Plus A Control Tablet - 2 Pcs. 7) Phantom For Learning Infant Resuscitation - 1 Pc. 2. A Detailed Description Of The Subject Of The Order, Including An Indication Of The Minimum Parameters And Technical, Functional And Operational Requirements, Is Included In Annex No. 2 To The Swz - Description Of The Subject Of The Order. 3. The Ordering Party Requires Products To Be Offered That Are New, Of Appropriate Quality, Free From Physical And Legal Defects And Having The Legally Required Standards And Certificates, As Well As Those Approved For Trading And Use In Poland, In Accordance With Applicable Law, And Meeting The Quality And Technical Standards Resulting From Their Function And Purpose. 4. The Subject Of The Contract Must Meet All The Requirements Of A Safe Product In Accordance With Regulation (Eu) 2023/988 Of The European Parliament And Of The Council Of 10 May 2023 On General Product Safety, Amending Regulation (Eu) No 1025/2012 Of The European Parliament And Of The Council And Directive (Eu) 2020/1828 Of The European Parliament And Of The Council And Repealing Directive 2001/95/Ec Of The European Parliament And Of The Council And Council Directive 87/357/Eec Of 10 May 2023 (Oj Eu No. 135, P. 1). The Contractor Is Obliged To Deliver A Brand New Subject Of The Order (In Particular, Unrepaired, Unrefabricated, Not Exhibited, Previously Unused) From Current Production And A Legal Distribution Channel, Efficient, Complete (In Particular With All Components, Parts, Materials) And Compliant With The Opz. 5. Warranty Period: Min. 24 Months. The Warranty Period Is One Of The Criteria For Evaluating Offers. 6. The Ordering Party Requires Products With The Same Or No Worse Parameters And Quality Standards As Indicated In Annex 2 To The Swz. Failure By The Contractor To Confirm And Meet At Least One Of The Required Technical Parameters In The Table Of Compliance Of The Offered Equipment With The Subject Of The Order (Appendix 1A To The Swz) Will Result In Rejection Of The Offer. 7. Wherever The Swz And Its Annexes Contain Terms Indicating Trademarks, Patents Or Origin, Sources Or A Specific Process That Characterizes The Products Supplied By A Specific Contractor, Such An Indication Is Accompanied By The Words Or Equivalent, The Ordering Party Allows For The Possibility Of Contractors Offering Equivalent Products, Which Will Be Assessed On The Basis Of The Equivalence Assessment Criteria
