The Subject Of The Order Is The Collection And Transport To The Disposal Site Of Medical Waste And Expired Medicines. A Detailed Description Of The Subject Of The Order Is Set Out In Annex No. 2 To The Terms Of Reference. The Subject Of The Order Will Be Collected At Least Twice A Week On Days Agreed With The Ordering Party, Between 8:00 A.M. And 3:00 P.M., From The Ordering Partys Office At Ul. Woronicza 19 In Warsaw, With The Proviso That During The Period Of The Epidemic In Poland, The Contractor Will Be Obliged To Collect At Least 500 Kg Of Waste During Each Collection, Unless The Ordering Party Provides At Least In Advance. 2 Business Days Will Inform The Contractor In Writing About The Cessation Of The Obligation To Collect Waste In The Above-Mentioned. Minimum Quantity. It Is Allowed To Weigh Waste Using The Contractors Own Verified Device At The Time Of Waste Collection At The Ordering Partys Premises, However, The Ordering Party Reserves The Right To Control The Amount Of Waste Transferred Using Its Own Verified Device. The Costs Should Include Transport And Loading. Transport Must Be Carried Out In Vehicles Adapted To Transport Medical, Hazardous And Special Waste And Operated By Qualified Personnel, In Accordance With Applicable Legal Regulations. Medical Waste With Code 18 01 03* Will Be Transferred By The Ordering Party In Red, Tight, Plastic And Tied Bags. Confirmation Of The Received Quantity In The Form Of A Waste Transfer Card In Electronic Form On The Products And Packaging And Waste Management Database Platform. Principle Of Proximity: In Accordance With Art. 20 Section 3 And Art. 6 Of The Act Of December 14, 2012 On Waste (Consolidated Text: Journal Of Laws Of 2023, Item 1587, As Amended) - It Is Prohibited To Dispose Of Infectious Medical Waste Outside The Voivodeship In Which It Was Generated. Infectious Medical Waste May Be Disposed Of In A Voivodeship Other Than The One In Which It Was Generated, In The Nearest Installation, If There Is No Installation For The Disposal Of This Waste In A Given Voivodeship Or If The Existing Installations Do Not Have Spare Processing Capacity. The Contractor Undertakes To Perform The Subject Of The Order In Accordance With All The Requirements Of The Ordering Party Indicated In The Tor. If The Description Of The Subject Of The Contract Includes References To Standards, Technical Assessments, Technical Specifications And Technical Reference Systems Referred To In Art. 101 Section 1 Point 2 And Section 3 Of The Public Procurement Law, The Ordering Party Allows Solutions Equivalent To Those Described.
