The Subject Of The Order Is Reconstruction Of Farm Buildings For The Complex Of Specialist Outpatient Clinics Of The Małopolska Orthopedic And Rehabilitation Hospital Named After Prof. Bogusław Frańczuk Covering The Aos Area And The Creation Of A One-Day Surgery Department. 2. The Order Includes: - Development Of Design Documentation Along With Obtaining The Necessary Construction Permits; - Performance Of Construction And Installation Works, Including The Delivery And Assembly Of Equipment; - Obtaining Acceptance Of The Entire Investment Process In Including Final Acceptance; - Obtaining Acceptance Of The Facility For Use. The Order Is Carried Out In The Design And Build Formula. Stage 1 - Includes The Preparation Of Complete Design Documentation, Including Obtaining All Required Permits And Administrative Decisions Necessary For The Implementation Of Construction Works, And Conducting An Accessibility Audit; Commissioning Of A Brand New Digital X-Ray Device), As Well As Obtaining The Final Permit For Use And Execution Of Other Elements Of The Subject Of The Contract Not Covered By Stage 1. 4. The Provisions Of The Swz Should Be Read Together With Any Changes To The Content Of The Swz, E.G. As A Result Of The Responses To The Contractors Inquiries. 5. The Ordering Party, Within The Limits Specified In The Provisions Of Article 462 Et Seq. Of The Public Procurement Law, Allows The Contractor To Perform The Subject Of The Contract Using Subcontractors. The Contractor Who Intends To Entrust The Performance Of Part Of The Order To Subcontractors Shall Indicate In The Offer The Parts Of The Order The Performance Of Which He Intends To Entrust To Subcontractors, And Provide The Names Of Possible Subcontractors - If They Are Already Known. 6. In The Light Of Article 95 Of The Public Procurement Law, The Ordering Party Requires The Contractor (Or Subcontractor) To Employ Persons Performing Activities, I.E. Manual Workers Carrying Out Construction, Installation And Assembly Works, Auxiliary Works, And Machine Operators On The Basis Of An Employment Relationship. The Activity Involves The Provision Of Work In The Manner Specified In Article 22 § 1 Of The Act Of June 26, 1974 - Labor Code (Consolidated Text: Journal Of Laws Of 2025, Item 277, As Amended). 7. The Ordering Party Does Not Allow The Submission Of Variant Offers. 9. The Ordering Party Did Not Conduct Preliminary Market Consultations. 10. The Ordering Party Allows The Contractors To Visit The Investment Site (The Site Visit Is Optional). The Dates Of The Site Visits Are Indicated In The Swz.
