The Subject Of The Order Is The Provision Of Services To The Ordering Party In The Field Of Conducting Sensory Integration Therapy Classes In Order To Implement The Tasks Specified In Art. 90V Section 4 Of The Act Of September 7, 1991 On The Education System And In The Regulation Of The Minister Of Education And Science Of August 30, 2023 On The Detailed Tasks Of Leading Coordination, Rehabilitation And Care Centers (Journal Of Laws Of 2023, Item 1801).2. Services In The Field Of Sensory Integration Therapy Classes Will Be Provided In The Form Of Individual Or Group Classes With Children From Birth Until They Start School. In The Case Of Group Classes, They Will Be Conducted With A Group Of No More Than Three Children.3. The Duration Of Classes Is 1 Hour, I.E. 60 Minutes.4. Place Of Classes: Special School Complex, Ul. Kopernika 2, 39-40 Tarnobrzeg Or Other Place Indicated By The Ordering Party In The City Of Tarnobrzeg (Including The Therapy Participants Home).5. Deadline For Classes: From The Date Of Signing The Contract To December 15, 2026. During A State Of Epidemic Threat, A State Of Epidemic, In Particular In Connection With Sars-Cov-2 Virus Infections Or In The Case Of Emergency States In The Territory Of The Republic Of Poland Announced In Particular In Connection With Sars-Cov-2 Virus Infections, The Service, At The Express Request Of The Parents/Legal Guardians Of The Therapy Participant, May Be Provided Remotely, In Compliance With The Appropriately Applied Principles And Rules Regarding The Service Provided Directly.6. Number Of Hours To Be Completed: Max. 800 Hours7. The Schedule Of Classes Will Be Determined By The Contractor, Taking Into Account The Needs Of The Child And Parents And Taking Into Account The Work Schedule In Force At The Place Where The Classes Are Held (Zss In Tarnobrzeg), Classes Will Be Held From Monday To Saturday.8. Settlement For The Services Provided Will Be Monthly, Based On Invoices And A Monthly Summary Of Hours Completed Under The Program. The Tasks Of The Sensory Therapy Instructor Will Include: • Conducting Therapy For Developmental Disorders In Children Using Sensory Integration Methods • Conducting Sensory Integration Therapy For Children Based On Opinions From A Psychological And Pedagogical Clinic, Medical Judgments And Certificates, Si Diagnosis, • Developing Individual Forms Of Therapeutic Support For Participants, Programs, • Cooperation With The Therapeutic Team Regarding Methods And Work Technicians, • Conducting Consultations And Providing Professional Therapeutic Assistance To The Project Participant And His/Her Guardians, • Consultations With Other Specialists Of The For Life Program, Teachers And Parents. • Keeping Documentation Of Classes, Including: Keeping Class Diaries, Keeping Time Sheets According To The Provided Template, Ongoing Cooperation With The Ordering Party, Providing The Ordering Party With The Following Documents Regarding The Implementation Of Classes: A Monthly Time S
