Speech Therapy Services - Neurologopedia. Lot No. 1 - Covers Approximately 400 Hours, Place Of Service In Świdnica Or The Świdnica Commune. Cpv 80340000-9 Special Education Services 85140000-2 Various Services In The Field Of Health. The Subject Of The Order Is The Provision Of Speech Therapy Services - Neurologopedia For Children Aged 0 To Start Primary School Education As Part Of The Comprehensive Support Program For Families For Life. As Part Of The Service, The Leader/Service Provider Is Obliged To Take Part In Meetings, Workshops, And Possibly Additional Team Work Commissioned By The Ordering Party. The Planned Schedule Of Classes Will Be Divided Into The Following Periods: - 1St Period From The Date Of Signing The Contract - Until April 2026; - 2Nd Stay From May 2026 - Until August 2026 - 3Rd Stay From September 2026 - Until December 2026. Performers Availability: Assuming: • 1-5 Children Per Week, The Performer Must Be Available At Least Once (One Day) A Week;• 6-12 Children Per Week, The Performer Must Be Available At Least Twice (Two Days) A Week;• 13-20 Children Per Week, The Performer Must Be Available Minimum Three Times (Three Days) A Week;• 21-28 Children A Week The Contractor Must Be Available A Minimum Of Three Times (Three Days) A Week;• 29-33 Children A Week The Contractor Must Be Available A Minimum Of Four Times (Four Days) A Week. The Contractors Responsibilities Will Include: 1) Diagnosing Problems, 2) Conducting Speech Therapy, 3) Keeping Documentation, Including: Specialists Work Sheet, Work Sheet With A Client, 4) Keeping A Register Of Admitted Persons, 5) Perform The Activities That Are The Subject Of The Contract With Due Diligence, 6) Supervise The Correct Implementation Of This Order, 7) Provide Services In Accordance With The Schedule, 8) Submit By The 5Th Day Of The Month A Summary Of The Support And/Or Therapy Provided, Taking Into Account: The Number Of Hours Allocated For Speech Therapy And The Number Of People Who Benefited From It. A Detailed Description Of The Subject Matter And Terms Of The Order Execution Is Included In The Draft Contract Constituting An Annex To The Terms Of Reference.
