The Ordering Party Commissions And The Contractor Accepts The Subject Of The Contract (Order), Which Is The Provision Of Services By One Assistant For An Elderly Person To The Participants Of The Project Entitled: Wielkopolskie Telecentrum Care (Hereinafter Referred To As The Project) In Their Place Of Residence In The Manner And Under The Terms Specified In This Contract.2. By The Project Participants Referred To In Section 1, Should Be Understood As Elderly People Over 60 Years Of Age From The City Of Turku Who, Due To Age, Health Or Disability, Require Care Or Support In Everyday Functioning, Hereinafter Referred To As Participants Or Participant.3. The Scope Of Duties Of An Elderly Persons Assistant Includes, Among Others: A. Development And Implementation Of A Work Plan With An Older Person In Consultation With The Coordinator Of The Senior Support Center And A Social Worker.B. Periodically Assessing The Situation Of An Elderly Person, At Least Every Six Months, And Submitting This Assessment To The Coordinator Of The Senior Support Center. C. Assistance With Hygienic Care (Washing, Bathing, Combing, Etc.) D. Changing Underwear, Bed Linen, Making The Bed.E. Ensuring The Cleanliness Of The Patients Underwear, Bedding And Clothing. F. Assistance In Meeting Physiological Needs.G. Maintaining Cleanliness Of The Rooms Used By The Person Receiving Assistance By Performing Routine Cleaning.H. Shopping For Groceries And Other Household Products.I. Preparing Meals Taking Into Account The Diet, Including One Hot Meal.J. Delivering Fuel From The Storage Site And Burning It In The Furnace (If Necessary).K. Ordering Medical Appointments, Contacting A Doctor On Health Matters, Accompanying During Doctors Visits, Filling Prescriptions.L. Coordination Of Activities Of A Doctor, Community Nurse, Social Worker, Caregiver In A Social Welfare Center, Family And Other People Involved In The Care Of An Elderly Person. M. Dealing With Current Matters According To The Justified Needs Of The Ward, E.G. Payment For Care Services Provided, Payment For Apartments, Utilities, Assistance In Handling Official Matters, Etc. N. Maintaining Individual Interests And Helping In Meeting Spiritual Needs (E.G. Delivering Newspapers, Books, Reading, Taking People To The Cemetery Or Church, Organizing Contacts With Neighbors, Encouraging Participation In The Senior Club, The University Of The Third Age, Maintaining Contacts With The Environment And Family). Improving The Mobility Of An Elderly Person (Social Inclusion Through Joint Trips To The City For Shopping, To The Doctor, To The Cemetery To The Family To Deal With Current Matters) P. Emotional Support (Organizing And Spending Free Time Together, Talking)Q. Respite Help (Support For Carers Of Dependent People - Replacing A Family Member Taking Care Of A Dependent Person So That They Can Take Care Of E.G. Current Matters) Notifying The Coordinator Of The Senior Support Center About The Possible Needs Of The Person Under Care
