Purchase Of Veterinary Services - Part A -1.Pilaszaków, 2.Pgr Pilaszków, 3.Myszczyn, 4.Orły, 5.Pogroszew 6.Kolonia, 7.Pogroszew, 8.Umiastów, 9.Pgr Wolskie, 10.Wolskie, 11.Michałów, 12.Koprki, 13.Ołtarzew, 14.Ożarów Mazowiecki Precincts (0001, 0002, 0003).4.3 The Implementation Of The Subject Of The Order In The Scope Of Parts A - C Includes The Following Procedures Or Tests: 1) Sterilization/Neutering (Including Abortion Sterilization), 2) Obligatory Post-Treatment Care In The Case Of Free-Living Cats, 3) Euthanasia Of Blind Litters, 4) Stationary Treatment Including Medical Procedures, 5) Euthanasia, 6) Each Check Of The Chip Number, 7) Stay In The Clinic, Including The Maintenance Of The Animal,8) Flea And Deworming,9) Fiv/Felv Tests - In The Case Of Cats,10) X-Ray Examination,11) Ultrasound Examination,12) Complete Blood Count - Blood Test,13) Stool Examination,14) Vaccination Against Rabies,15) Vaccination Against Infectious Diseases4.4 The Ordering Party Also Reserves That The Place Of Provision Of Veterinary Services May Be Distant From The Administrative Boundaries Of The Ożarów Commune Mazowiecki By A Maximum Of 5 Km, And This Distance Should Be Measured Along The Shortest Route That The Vehicle Will Cover While Driving On Public Roads Between The Address Indicated As The Place Of Service Provision And The Border Of The Ożarów Mazowiecki Commune. The Contractor Will Submit An Appropriate Declaration In This Regard In The Offer Form. 4.8 The Subject Of The Order For Parts A, B, C, D Should Be Implemented In Accordance With The Description Of The Subject Of The Order According To Annex No. 1 To Swz.4.9 Cpv Codes: - 85200000-1 - Veterinary Services4.10 The Ordering Party Will Inform The Contractor About Any Changes During The Execution Of The Contract. However, The Contractors Remuneration Resulting From The Implementation Of The Subject Of The Order May Not Exceed The Financial Resources Secured In The Ordering Partys Budget.4.10.1 The Ordering Party, Pursuant To Art. 95 Of The Public Procurement Law, Requires The Contractor Or Subcontractor To Employ, On The Basis Of An Employment Relationship, Persons Performing Activities Specified By The Ordering Party In The Scope Of Contract Execution, If The Performance Of These Activities Involves Performing Work In The Manner Specified In Art. 22 § 1 Of The Act Of June 26, 1974, Labor Code (Journal Of Laws Of 2023, Item 1465). 4.10.2 The Requirements For Employment By The Contractor Or Subcontractor Under An Employment Relationship Apply To Persons Performing Activities Directly Related To The Implementation Of The Subject Of The Order - This Obligation Does Not Apply To Situations Where These Works Will Be Performed Independently And Personally By Natural Persons Running A Business In The Form Of The So-Called Self-Employment As A Subcontractor.4.10.3 The Method Of Verification Of Employment, As Well As The Ordering Partys Rights To Control The Contractors Compliance With Employment Requirements A
