Partial Task No. 1: Conducting Physiotherapy/Exercise Rehabilitation Classes Estimated Number Of Hours 2,851. The Subject Of The Order Is A Service Consisting In Conducting Early Child Development Support Classes And Specialist Classes For Children With Disabilities And Those At Risk Of Disability, Aged 0 To The Moment The Child Starts School, In The Limanowa District As Part Of A Government Administration Task Resulting From The Comprehensive Support Program For Families For Life, Constituting An Annex To Resolution No. 189 Of The Council Of Ministers Of December 27, 2021 Amending The Resolution On The Comprehensive Support Program For Families Za Życiem (M.P. Of 2022, Item 64), Hereinafter Referred To As The Programme, In The Scope Of Implementing The Tasks Of The Leading Coordination, Rehabilitation And Care Center In The Poviat, Specified In Art. 90V Section 4 Of The Act Of September 7, 1991 On The Education System And In The Regulation Of The Minister Of National Education Of September 5, 2017 On The Detailed Tasks Of Leading Coordination, Rehabilitation And Care Centers - Task 2.4 Of The Program: Multi-Faceted And Comprehensive Assistance To A Disabled Child From The Age Of 0 To Starting School And His Family, For The Special Center School And Educational Center In Dobra.2. The Number Of Hours To Be Completed In Each Part Of The Order Has Been Given As Maximum. The Ordering Party Reserves The Right To Reduce The Number Of Hours Under Each Part Of The Order, Depending On The Number Of Children Referred To The Program And Their Diagnosed Therapeutic Needs. Billing Is Made Monthly Based On Hours Actually Completed. In Relation To Each Part Of The Order, The Ordering Party Will Assign The Contractor The Number Of Hours In Accordance With The Demand, Not Less Than 75% Of The Number Of Hours In A Given Part Of The Order (Rounded Down To An Equal Number Of Hours). 3. Services Will Be Provided In The Form Of Individual Classes With Children From The Age Of 0 To Starting School And With Their Families.4. The Duration Of Classes Is 1 Hour, I.E. 60 Minutes. 5. The Contractor Will Provide Rooms/Offices For Conducting Classes And Will Bear The Costs Associated With Their Maintenance, Taking Into Account The Accessibility Requirements Referred To In Point 3.11. The Contractor Provides Equipment And Teaching Aids Necessary To Conduct The Classes.6. A Detailed Description Of The Subject Of The Contract And The Terms Of Execution Of The Contract Are Included In The Description Of The Subject Of The Contract And In The Draft Provisions Of The Contract Constituting Annexes To The Swz.
