1. The Subject Of The Order Is The Provision Of Services In The Field Of Collection, Transport Of Corpses To A Cold Store And Their Storage, Washing, Clothing And Carrying Out Autopsies Or Examinations.2. The Service Includes:1. Collection Of Patients Bodies From Hospital Wards Or Another Place Indicated By The Ordering Party After 2 Hours From The Time Of Death. The Contractor Must Provide Bags Suitable For Transporting Bodies At His Own Expense. Transport Of Bodies On A Stretcher Intended For Transporting Bodies (The Contractor Is Obliged To Have Them);2. Performing An Autopsy At The Request Of The Ordering Party By A Doctor With The Qualifications Required In This Area (Within 12 Hours From The Date Of Death And Within 72 Working Hours From The Date Of Death) And Documenting It In Accordance With Applicable Regulations. The Doctor Performing The Autopsy After Its Completion Is In Particular Responsible For: A) Immediately Establishing An Anatomopathological Diagnosis And Entering, Together With The Icd-10 Codes, In The Medical History Of The Autopsied Patient, And Then After Authorization Entries Made, Immediate Transfer Of The Medical History In A Manner Ensuring Confidentiality And Data Protection To The Sales Department - Medical Statistics Unit At The Ordering Partys Headquarters, B) Preparing Two Copies Of The Autopsy Report With The Finally Established Anatomopathological Diagnosis, One Copy Of Which Is Forwarded To The Ordering Party To Be Included In The Medical History, C) Issuing A Death Certificate In A Situation Where The Autopsy Examination Is Carried Out For Reasons Specified In The Code Of Criminal Procedure And The Implementing Penal Code And The Acts Implementing Regulations Issued On Their Basis.3. Transporting The Corpse To The Mortuary Using The Contractors Transport;4. Performing Post-Mortem Toilet For Patients (Washing, Dressing);5. Storing The Bodies Of Deceased Patients In A Cold Store For No Longer Than 72 Hours, Unless: A) They Cannot Be Collected Earlier By Persons Authorized To Bury The Deceased Person, Referred To In Art. 10 Of The Act Of January 31, 1959 On Cemeteries And Burial Of The Dead (Journal Of Laws Of 2025, Item 1590), B) An Inquiry Or Investigation Was Initiated In Connection With The Death, And The Prosecutor Did Not Allow The Burial Of The Body, C) There Are Important Reasons Other Than Those Mentioned Above, With The Consent Or At The Request Of The Person Authorized To Bury The Deceased Person.6. Free Delivery Of Patients Bodies In Accordance With Applicable Regulations And Documentation To A Person Entitled To Burial In A Condition Ensuring The Preservation Of The Dignity Due To A Deceased Person.7. The Ordering Party Does Not Bear The Costs Of Storing The Bodies Of Deceased Patients In A Cold Store For More Than 72 Hours.8. The Family Or Persons Authorized To Prepare The Burial Have A Free Choice Of The Company Providing Further Comprehensive Funeral Services, The Costs Of Which Are Borne By The Fa
