1. The Subject Of The Order Is A Service Consisting In The Provision Of Specialized Care Services Pursuant To The Act Of March 12, 2004 On Social Assistance. The Act Of March 12, 2004 On Social Assistance (Consolidated Text: Journal Of Laws Of 2025, Item 1214, As Amended) And The Regulation Of The Minister Of Social Policy Of September 22, 2005 On Specialist Care Services Regulation Of The Minister Of Social Policy Of September 22, 2005 On Specialist Care Services (Consolidated Text: Journal Of Laws Of 2024, Item 816) Ensuring Access To Rehabilitation, Revalidation And Educational Activities For Children And Adolescents With Mental Disorders, In Exceptional Cases, If They Are Unable To Obtain Access To The Activities Referred To In Art. 7 Of The Act Of August 19, 1994 On Mental Health Protection (Consolidated Text: Journal Of Laws Of 2024, Item 917) In Their Place Of Residence In Accordance With Medical Recommendations Or Other Indications Justifying The Specific Needs Of The Centers Residents. The Type Of Services, The Scope Of Services, The Period Of Service Provision, And The Number Of Hours Will Be Determined Individually For Each Child In The Administrative Decision, Which Constitutes The Basis For The Provision Of Services For Each Specialist. In Order To Ensure The Highest Quality Of Services And Taking Into Account The Well-Being Of Each Child, It Is Recommended That The Maximum Number Of Services Be Provided To Each Child By 1 Specialist In A Given Part Of The Order. The Subject Of The Order Is Divided Into 2 Parts, Consisting In The Provision Of Services: Part 1: Oligophrenopedagog. The Ordering Party Reserves The Obligation For The Contractor To Personally Perform Key Parts Of The Order Consisting In The Provision Of Specialized Services: Part 1: Oligophrenopedagog2. The Ordering Party Described The Subject Of The Order Using: • Description Of The Subject Matter Of The Order (Appendix No. 2 To The Swz) • Draft Provisions Of The Agreement (Appendix No. 3 To The Swz) 3. Order Completion Date: 10 Months, With The Possibility Of Extension To 12 Months No Longer Than Until December 24, 2026, Subject To The Ordering Party Exercising Its Option Right.
