The Subject Of The Order Is The Ongoing Maintenance Of Improved Ground Surfaces On Municipal Roads In The City Of Starachowice In Accordance With The Following Scope: 1) Current Maintenance Of Ground Surfaces Using A Wedge With An Average Pothole Depth Of Up To 8 Cm, Taking Into Account The Cost Of Purchase And Transport Of Road Material: Quantity (Only For The Purposes Of Determining The Offer Price) - Minimum 3,000 M2 - Maximum 4,000 M2; 2) Ongoing Maintenance Of Ground Surfaces Using Crushed Stone With An Average Pothole Depth Of Up To 8 Cm, Taking Into Account The Cost Of Purchasing And Transporting Road Material: Quantity (Only For The Purpose Of Determining The Offer Price) - Minimum 350 M2 - Maximum 500 M2; 3) Ongoing Maintenance Of Ground Surfaces Using Reclaimed Asphalt With A Pothole Depth Of Up To 8 Cm, Taking Into Account The Cost Of Purchasing And Transporting Road Material: Quantity (Only For The Purposes Of Determining The Offer Price) - Minimum 4,500 M2 - Maximum 6,000 M2; 4) Ongoing Maintenance Of Ground Surfaces Using Other Material Owned By The Ordering Party (Including: Reclaimed Asphalt, Aggregate From Concrete Rubble) With A Pothole Depth Of Up To 8 Cm, Taking Into Account The Cost Of Loading And Transport Within The Administrative Limits. Cities (Average Distance Approx. 5 Km): Quantity (Only For The Purpose Of Determining The Offer Price) – Minimum 350 M2 – Maximum 500 M2; 5) Mechanical Leveling Of Ground Surfaces Using A Grader: Quantity (Only For The Purposes Of Determining The Offer Price) - Minimum 50,000 M2 - Maximum 67,000 M2. All Works Related To The Execution Of The Subject Of The Order Should Be Performed In Accordance With The Annex To The Swz Entitled: Requirements For The Execution, Conduct And Acceptance Of Works Related To The Current Maintenance Of Road Surfaces With Improved Soil Surfaces. Services Related To The Current Maintenance Of The Surface Will Be Performed On Specified Dates, In Accordance With The Instructions Of The Ordering Partys Representative. The Contractor Is Obliged To Provide Passenger Transport For The Joint Weekly (Maximum 4 Hours A Week) Inspection Of Municipal Roads With The Ordering Partys Representative In Order To Carry Out Disposal, Inspection And Acceptance Activities. The Contractor Bears Full Civil Liability For Damage Caused By Its Own Actions Or Omissions Related To The Implementation Of This Order, As Well As The Actions Or Omissions Of Persons Used By The Contractor To Perform Order. The Ordering Party Requires Employment By The Contractor Or Subcontractor On The Basis Of An Employment Relationship, Within The Meaning Of Art. 22 § 1 Of The Act Of June 26, 1974, Labor Code, Persons Performing The Indicated Activities Related To The Implementation Of The Order, I.E. Carrying Out Ongoing Maintenance Of Ground Surfaces And Activities Related To The Use Of Machines And Devices In Road Works Related To The Implementation Of The Order.
