1. The Subject Of This Order Is The Comprehensive Provision Of Laundry Services For Hospital Linen, Including Transport, For A Period Of 12 Months From Signing The Contract, But Not Earlier Than February 4, 2026, In The Following Quantities:• Medical Underwear - 85,000 Kg• Patients Underwear - 21,600 Kg 2. A Detailed Description Can Be Found In Annex No. 1 To Swz - Description Of The Subject Of The Order And In The Annex No. 3 To Swz - Price Form.3. The Subject Of The Contract Is Defined In The Common Procurement Vocabulary With The Following Code: Cpv Code 98310000-9; 98311000-6; 98315000-44. The Ordering Party Does Not Allow The Submission Of Partial Offers. The Lack Of Division Into Parts Of The Order Is Caused By The Possibility Of Excessive Difficulties Or Costs In The Implementation Of The Service. Division Of The Order Due To Its Size Is Not Justified. The Division Could Lead To The Lack Of Offers For One Or More Parts, Which In Turn Would Contribute To The Extension Of The Procedure And The Implementation Of The Task.5. The Ordering Party Does Not Allow The Submission Of Variant Offers. 6. The Ordering Party Does Not Envisage The Possibility Of Awarding The Contract Referred To In Art. 214 Section 1 Point 7 And 8 Of The Public Procurement Law. 7. Employment Requirements Based On An Employment Relationship In The Circumstances Referred To In Art. 95 Of The Act: 1) The Ordering Party Requires The Contractor Or Subcontractor To Employ, Under An Employment Relationship, Persons Performing The Following Activities Within The Scope Of The Order: I.E. Activities Related To Collecting Laundry From The Ordering Party. 2) During The Execution Of The Order, The Ordering Party Is Entitled To Carry Out Inspection Activities Towards The Contractor Regarding Compliance By The Contractor Or Subcontractor With The Requirement Of Employing Persons Performing The Activities Specified In Point 1 On The Basis Of An Employment Relationship. In Order To Verify The Fulfillment Of These Requirements, The Ordering Party Is Entitled, In Particular, To Request: A) A Declaration Of The Employed Employee, B) A Declaration Of The Contractor Or Subcontractor On The Employment Of The Employee Under An Employment Contract, C) A Certified Copy Of The Employment Contract Of The Employed Employee, D) Other Documents - Containing Information, Including Personal Data, Necessary To Verify Employment Under An Employment Contract, In Particular The Name And Surname Of The Employed Employee, The Date Of Conclusion Of The Employment Contract, The Type Of Employment Contract And The Scope Of The Employees Responsibilities;3) The Contractor, At Each Request Of The Ordering Party, Will Submit To The Ordering Party The Information Indicated In Point Within The Time Limit Specified In The Request. 2 Evidence To Confirm The Fulfillment Of The Employment Requirement Under An Employment Relationship By The Contractor Or Subcontractor Of Persons Performing The Activities Listed In Point 1.
