1. The Subject Of The Order Is The Provision Of Services Of A Personal Assistant To A Person With A Disability In 2026 To The Residents Of The Wejherowo Commune Under The Personal Assistant To A Person With A Disability Program For Local Government Units - 2026.2 Edition. The Aim Of The Service Is To Provide Support In The Form Of A Personal Assistant To A Person With A Disability, I.E. Support In Performing Everyday Activities And Functioning In The Social Life Of People With Disabilities. 3. To The Extent Not Regulated In This Documentation, The Personal Assistant For A Person With Disability Program For Local Government Units - 2026 Edition, Constituting Annex No. 9 To The Terms Of Reference (Hereinafter Referred To As The Programme) Applies. 4. The Provision Of Services In The Form Of A Personal Assistant To A Person With A Disability Is Planned For 26 People With Disabilities, Residents Of The Wejherowo Commune, Including: 1) 8 Adults With A Certificate Of A Significant Degree Of Disability And Combined Disability, For A Total Of 3,690 Hours; 2) 12 Adults With A Certificate Of A Significant Degree Of Disability, For A Total Of 2,060 Hours; 3) 5 Children Aged 2 Years And Over Years Of Age Up To 16 Years Of Age Meeting The Provisions Of Part Iii Of The Program, For A Total Of 1,250 Hours;4) 1 Disabled Person With A Certificate Of A Moderate Degree Of Disability With Multiple Disabilities, For A Total Of 300 Hours. 5. The Estimated Total Amount Of Personal Assistance Services Is 7,300 Hours. 6. The Maximum Number Of Hours Given Is An Estimate (The Maximum Limits Of Hours Financed From The Solidarity Fund Are Determined By The Program). In Relation To The Maximum Number Of Hours, The Ordering Party Stipulates That: 1) It Is Possible To Reduce The Maximum Number Of Hours Of Services Provided By A Personal Assistant To A Person With A Disability In Accordance With The Needs And Financial Resources Received By The Ordering Party And To Proportionally Reduce The Financial Resources Allocated To The Implementation Of The Contract; 2) If The Number Of Hours Performed Is Less Than The Maximum, The Ordering Party Will Pay The Contractor For The Actual Number Of Hours Of Services Provided By A Personal Assistant To A Person With A Disability; 3) If The Funds Allocated For The Execution Of The Order Are Exhausted Earlier Than The Deadline For Completion Contract, This Results In Earlier Termination Of The Contract; 4) The Ordering Party Sets The Minimum Number Of Hours To Be Completed During The Contract Period At 5,900 Hours. The Minimum Size Is The Guaranteed Amount Of Services For The Contractor.7. The Place Of Provision Of Services Is The Place Of Residence Of The Disabled Person Located In The Wejherowo Commune.8. For The Purposes Of This Order, It Is Assumed That 1 Hour Of Service Means 60 Minutes Of Direct Service Provision To The Client.
