The Subject Of The Order Is The Provision Of Shelter Services For Homeless People Whose Last Place Of Residence Is The City Of Zgierz (Based On Article 101(2) Of The Act Of March 12, 2004 On Social Assistance).1. The Ordering Party Requires That The Shelter Be Located Within A Radius Of Up To 30 Km From The Seat Of The Zgierz City Hall, Pl. Jana Pawła Ii 162. Referring A Person To A Shelter Is Based On An Administrative Decision Granting Assistance In The Form Of Shelter.3. Placement Of Homeless People In Emergency Cases, I.E. Without A Referral, May Take Place After The Centers Opening Hours And On Days Off. The Referral Will Be Issued Immediately After Conducting A Family Community Interview And Completing The Documents Of The Person Concerned.4. Required Scope Of Services Under The Subject Of The Order: 1) 24-Hour Shelter, Accommodation And Temporary Check-In For Homeless People, Including Women And Men, Providing Their Own Bed And Bedding (Mattress, Pillow, Duvet Or Blanket, Duvet Cover And Sheet) And Permanent Access To A Toilet And Bathroom With Hot And Cold Water (In Accordance With The Regulation Of The Minister Of Family, Labor And Social Policy On Minimum Standards Of Night Shelters, Shelters For Homeless People, Shelters For Homeless People With Services Care And Warming Rooms Of July 28, 2025), 2) Providing A Room For Isolation Of Newly Admitted Persons (In Special Cases), 3) Meals In The Form Of At Least Three Meals A Day, Including At Least One Hot Meal, 4) Providing Cleaning And Personal Hygiene Products, And, If Necessary, Delousing Agents, 5) Providing The Necessary Clothing (Clothing And Footwear) Appropriate To The Season And Physical Characteristics Of The Homeless Person, 6) Assistance In Formal And Legal Matters, 7) Providing Basic Over-The-Counter Medicines And Dressings As Well As Access To A General Practitioner And Specialists,8) Carrying Out Activities Supporting The Process Of Residents Independence, Including: Based On Individual Programs For Overcoming Homelessness Developed By Social Workers.5. Average Monthly Number Of People Requiring Shelter Estimated Based On Data From 2024/2025. Is 16 In The Winter Months 24 People.6. The Contractor Ensures That A Hospitalized Person Can Maintain A Place In The Shelter And Provides Assistance In The Form Of Hygiene Products And Clothing, And The Period Of Stay In The Hospital Does Not Release His/Her Place In The Facility.7. The Cost Of Stay Of A Person Staying In A Shelter Who Is Subject To Hospitalization: A) Up To 3 Days Of Hospitalization Without Affecting The Fees Incurred, B) Over 3 Days Of Hospitalization - The Fee Will Be Charged In The Amount Of Half The Daily Rate.8.
