1. The Subject Of The Order Is The Provision Of A Pharmaceutical Service Consisting In The Preparation Of Mixtures For Parenteral Nutrition, Made On The Basis Of Individual Prescriptions, Along With The Delivery Of The Finished Mixture In The Form Of Bags According To The Quantitative And Generic List Specified In Annex No. 2 To The Specification Of The Terms Of Order, Hereinafter Referred To As Swz, For The Neonatology Department Of The Provincial Hospital In Bielsko-Biała, Al. Armii Krajowej 101, 43-316 Bielsko-Biała, Under The Conditions Specified In This Chapter And Under The Conditions Specified In The Draft Provisions Of The Agreement Constituting Annex No. 4 To Swz.2. The Subject Of The Order Will Be Performed At The Contractors Expense And Risk.3. The Mixtures Will Be Prepared In The Parenteral Nutrition Laboratory Of The Contractors Pharmacy Using A Mixer, Under A Bed With Laminar Airflow. The Fact That The Mixture Has Been Prepared Must Be Each Time Confirmed By An As-Built Report Attached To Delivery, Containing At Least The Following Data: 1) Batch Number Of The Mixture, 2) Name And Surname Of The Patient, 3) Date And Time Of Preparation Of The Mixture, 4) Volume Of The Mixture.4. The Contractor Is Obliged To Provide The Service In Accordance With The Provisions Of The Act Of September 6, 2001 - Pharmaceutical Law (Consolidated Text: Journal Of Laws Of 2024, Item 686, As Amended) And In Accordance With The Applicable Standards And Recommendations Of The Polish And European Societies Of Parenteral And Enteral Nutrition.5. The Contractor Is Obliged To Prepare Mixtures In Conditions Meeting The Requirements Of Good Manufacturing Practice (Gmp).6. The Mixtures Will Be Prepared On The Basis Of Personalized Prescriptions Provided To The Contractor By The Ordering Party.7. The Contractor Will Provide The Opportunity To Choose Preparations For Parenteral Nutrition, In Accordance With The List Below: 1) Glucose Solutions: 20%, 40% Or 5%; 2) Amino Acid Solutions: 10% Aminoven Inf., 10% Primene; 3) Aqua Pro Injectione; 4) Fat Emulsions: 10% Omegaven, 20% Smoflipid, 20% Intralipid; 5) Phosphates: Glycophos;6) Calcium: Organic Salts - 10% Calciumgluconicum, Inorganic - 10% Calciumchloratum;7) Electrolytes: 10% Nacl, 15% Kcl, 20% Mgso4;8) Trace Elements: Peditrace Or Nutryelt Pediatric;9) Vitamins (Soluble In Water And Fat): Soluvit N, Vitalipid N Infant. The Ordering Party Allows The Submission Of Offers Equivalent. The Ordering Party Will Consider An Offer Submitted For Preparations In A Form And Dose As Well As Quantitative And Qualitative Composition Corresponding To Those Indicated In This Section To Be An Equivalent Offer.8. The Mixtures Produced Must Meet Pharmacopoeial Requirements For Mixtures For Parenteral Nutrition.9. The Mixtures Produced Must Have Appropriate, Documented Stability (Physical, Chemical, Microbiological).10.
