Evidence of registration with Jigawa State Works Registration Board IV. Tax clearance certificate for the last three (3) years V. Audited Account for the Last One (1) Year VI. VAT Registration and Evidence of Past Remittance VII. Evidence of financial capability and banking support VIII. Companys Average Annual Turn Over of not less than N50 Million IX. That the Company have fulfilled all obligations to pay taxes, pensions, ITF Contribution and social security contributions X. Evidence of execution and completion of similar Projects at least 3 in the last 5 years XI. Verifiable evidence of Company profile and technical qualification of key personnel registered with relevant professional bodies (COREN, CORBON, QSRBN, ARCON etc. XII. A sworn affidavit certifying as Follows: a) That the document submitted for the pre-qualification exercise are not only genuine but correct b) That none of the directors of the Company has been convicted by any court of law c) That the Company is not bankrupt. d) That none of the officers of Jigawa State/Local Government is a present or former director of the Company. XIII Any other relevant information that will help in determining the Companys suitability for the work. SUBMISSION OF BIDDING DOCUMENT 1. The opening date is Tuesday, 11th November, 2025. 2. Companies are to submit their bids to the office of the Honourable Commissioner Ministry of Health on or before Tuesday, 11 November, 2025 3. A scanned copy of bid security of not less than 2% of the bid price either in form of insurance bond or bank guarantee addressed to the Honourable Commissioner, Ministry of Health Jigawa State, Dutse, Jigawa State. 4. Prequalification and financial bids should be submitted in two different sealed envelopes, (1 original and 2 photocopies in a bigger envelope) addressed to the Honourable Commissioner Ministry of Health, Block B New Secretariat Complex, Takur Dutse. 5. Similarly, the original of companys profile, signed forwarding letter with attached bill, bid security and a copy of the sworn Affidavit in hard copy should be send to Honourable Commissioner, Ministry of Health Jigawa State, Dutse, Jigawa State 6. Bids shall be valid for a period of 120 days (4month) after Bid opening. 7. Bids submitted after the deadline for submission will be returned un-opened. 8.
