The Subject Of The Investment Under The Task Titled Reconstruction Of Municipal Road 364905K At Km 0+000 To 0+620 In Szczawnica As Part Of The Task, The Following Works Should Be Performed: 1. Tearing Up The Existing Road Surface Made Of Concrete Blocks, Paving Stones And Multi-Hole Concrete Slabs On The Part Of The Sidewalk Intended For Reconstruction.2. Adjustment Of Concrete Road Edges.3. Leveling And Mechanical Compaction Of The Subgrade.4. Adjustment Of Sewage Grates And Wells.5. Laying The Road Surface With 15 Cm Thick Brushed Concrete In Formwork.6. Cutting And Filling Expansion Joints.7. Laying A Roadside Made Of Mechanically Compacted Crushed Aggregate With A Thickness Of 10 Cm And An Average Width Of 50 Cm At The Edge Of The Road Without A Sidewalk Or Hardened Parking Spaces.8. Complementing, Leveling And Thickening The Foundation With Adjustment Of The Concrete Edges Of The Pedestrian And Bicycle Path At Km 0+055 - 0+312 (In The Part Of The Sidewalk Intended For Reconstruction) With Widening Of The Section At Km 0+162 - 0+275 At The Expense Of The Road Width, Which Will Be Reduced To 3.5M On This Section.9. Laying Concrete Edges And Paving Stones (Without A Phase) On The Pedestrian And Bicycle Path Over A Length Of Approx. 260 M, Average Width 2.6 M. 10. Construction Of Textured Pedestrian Crossing Marking Using A Strip Of Integrated (Textured) Paving Stones At Km 0+112 And Km 0+214.11. Marking Pedestrian Crossings, Pedestrian And Bicycle Paths And Bus Bays With Horizontal And Vertical Signs.12. Installation Of 2 Street Lighting Lamps With A Power Supply Cable From The Existing Lamp, Approximately 80 M Long. 13. Installation - Relocation Of 2 Street Lighting Lamps Currently Located Close To The Edge Of The Road To Places Directly Next To The Wall Of The Garage Building, Together With An Extension Of The Power Supply Cable With A Length Of Approx. 2X3=6M.14. Installation Of 1 Piece Of Lighting Lamp With Asymmetric Luminous Flux Distribution At Km 0+112 And Installation Of 1 Piece Of Lighting Fixture With Asymmetric Luminous Flux Distribution On The Existing Lighting Pole At Km 0+214 At Pedestrian Crossings.15. Replacement Of 16 Spherical Lighting Fixtures With Directional Ones And Replacement Of Street Lighting Light Sources With Leds With An Equivalent Number Of Lumens.16. Construction Of 7 Parking Spaces By Hardening The Ground, Laying Concrete Edges And Paving Stones.17. Regulation Of Entrances To Properties And Surfaces At The Intersection, Adapting To The New Asphalt Surface.18. Installation Of A Board Informing About Co-Financing Of The Task On Steel Posts.19. Preparation And Submission Of A Complete Set Of As-Built Documentation. The Loop Length Of The Rebuilt Section Is Approx. 620 M, Width 3.5-6.1 M (Average 4.7 M). The Length Of The Walking And Cycling Path Is Approx. 257 M, Average Width 2.6 M. The Contractor Will Develop And Approve Temporary Traffic Organization. The Employer Allows For The Temporary Closure Of A Section Of The Road To Vehicular Traffic For The Time Necessary To Complete The Surface While Ensuring Access From Both Sides Of The Section Closed To Traffic. The Works Should Be Carried Out On The Basis Of A Notification Of Construction Works Not Requiring A Building Permit No. Ba.6743.1004.2023.Mr, Adopted Without Objection By The Starosta Of Nowy Targ On February 1, 2024, And Arrangements With The Investor Supervision Inspector. A Detailed Description Of The Subject Of The Order Is Set Out In Annex No. 4 To The Swz. Assumptions: 1. The Bills Of Quantities Were Treated By The Parties As Auxiliary Material For Estimating The Work Related To The Implementation Of The Subject Of The Contract, And In The Submitted Offer, The Contractor Included All Costs Related To The Implementation Of The Subject Of The Contract.2. As Part Of The Subject Of The Order, The Contractor Is Obliged To Perform Construction Works In Accordance With Construction Practice, Applicable Regulations And Standards, Based On The Prepared Documentation.3. The Contractor Will Ensure The Proper Performance Of All Works Related To The Implementation Of The Subject Of The Contract In Accordance With The Documentation And With Currently Applicable Standards, Construction Law Together With Implementing Acts And Other Applicable Regulations.4. Preparation Of As-Built Documentation In Accordance With Legal Regulations, Along With A Set Of Approvals, Certificates And Declarations Of Conformity For Built-In Materials And Devices.5. Completion Of The Subject Of The Contract Means The Performance Of All Works And Works Constituting The Subject Of The Contract.6. The Deadline For Completing The Subject Of The Contract Is The Date Indicated In The Final Acceptance Report Of The Subject Of The Contract, Signed By The Ordering Party And The Contractor.7. The Ordering Party Allows For Shortening The Deadline For Completing The Subject Of The Contract.8. The Contractor Is Obliged To Submit An Offer Cost Estimate No Later Than On The Day Of Signing The Contract. 9. No Later Than On The Date Of Signing The Contract, The Contractor Is Obliged To Submit A Declaration On The Method Of Invoicing In Accordance With The Variants Presented In § 11B* Of The Draft Provisions Of The Public Procurement Contract, Which Will Be Introduced Into The Content Of This Contract.
