The Subject Of A Classic Contract For Social Services In The Basic Mode - An Unlimited Tender Is The Purchase Of A Hot Meal (The So-Called Second Course) Along With The Service Of Serving It To Persons Indicated By The Employer In The Period From 03/07/2023 To 29/12/2023, Including :A) A Maximum Of 85 Meals A Day - A Maximum Of 10,710 Meals During The Term Of The Contract - On Working Days From Monday To Friday, Excluding Holidays And Holidays, Served At A Food Point In The Centrum District; B) A Maximum Of 25 Meals A Day - A Maximum Of 3,150 Meals During The Term Of The Contract - Along With The Service Of Delivery To The Customers Home In The City Of Tomasz w Mazowiecki On Working Days From Monday To Friday, Excluding Public Holidays And Holidays. If Any Of The Maximum Number Of Meals Specified Above Is Not Used, The Unused Number Of Meals Increases The Next Maximum Limit Of Meals.2. Place Of Serving Meals - A Place (Hereinafter Referred To As The Nutrition Point) That Meets The Sanitary And Hygienic Requirements, Approved By The Competent Sanitary Supervision Authority For Mass Catering, Equipped With A Toilet And A Washbasin (Equipped With Basic Sanitary And Disinfecting Products), Available Free Of Charge To Customers Pug. There Should Be A Place In The Premises To Leave Outer Clothing (Hangers). The Food Serving Point Must Be Located In A Heated Building With At Least 30 Seats (At Tables). In The Case Of Eating A Meal On Site, The Contractor Must Provide Customers With Their Own Ceramic Dishes And Metal Cutlery. Other Persons Entitled To A Hot Meal Will Receive Takeaway Meals In Disposable Dishes. The Contractor Shall Provide Disposable Dishes, Appropriate To The Type Of Meal, I.E. Dishes For The Second Course And Salad (All Portions Of The Second Course Must Be Packed Separately, I.E. Groats, Rice, Etc. Separately, Meat, Fish Separately, Portions Of Vegetables Separately). Disposable Dishes Should Have A Hygienic Certificate Of The Polish National Institute Of Hygiene, Allowing The Dishes To Come Into Contact With Food.3. A Hot Meal Should Be Served To Mops Clients On The Basis Of Lists Issued By The Ordering Party. It Is Not Allowed To Give A Meal To A Person Other Than A Mops Client Entitled To Receive It Without A Written Authorization To Collect The Meal Confirmed By A Social Worker Of The Local Centre.4. Meals Should Be Prepared In Accordance With Current Nutritional Standards And In Accordance With The Principles Of Collective Nutrition. The Contractor Will Prepare Meals In The Form Of A Second Course In Accordance With The Rules Set Out In The Act Of 25 August 2006 On Food And Nutrition Safety (Journal Of Laws Of 2022, Item 2123), Including The Implementing Provisions To The Above. Of The Act. Meals, In Terms Of Production Technology And Quality, Must Meet The Standards Set By The Food And Nutrition Institute, Which Is Part Of The National Institute Of Public Health, In Particular, Meals Must Have The Required Caloric Content And Must Be Prepared In Accordance With The Culinary And Sanitary Requirements For Mass Catering.5 . The Contractor Will Prepare Meals In Compliance With Sanitary And Epidemiological Requirements Regarding Personnel And Production Conditions And Will Take Responsibility For Their Compliance. 6. Persons Who Will Participate In The Performance Of The Contract Should Have A Medical Certificate For Sanitary And Epidemiological Purposes, As Defined By The Provisions On Infectious Diseases And Infections, On The Lack Of Contraindications To Perform Work Where There Is A Possibility Of Transferring The Infection To Other People, In Accordance With The Law.7 . A Hot Meal Will Be Served On Site From 11.00 To 14.00, At A Food Serving Point That Meets Sanitary Requirements In The City Of Tomasz w Mazowiecki In The Centrum District. Within The Meaning Of This Specification, The Centrum District Covers The Area Of The City Of Tomasz w Mazowiecki Located Between The Following Streets: Mo cickiego, Mireckiego, Nowy Port, Wodna, w. Antoniego, O. Lange, John Paul Ii, Podle na, Legions, Pi sudski, Gen. J zef Haller, Mostowa, Zawadzka, Orzeszkowa, Barlickiego, Warszawska, w. Antoniego, Graniczna, John Paul Ii, Podle na, General J zef Bem.8. Note: The Employer Establishes An Additional Tender Evaluation Criterion, I.E. Serving Meals For A Minimum Of 30 Mops Clients At The Second Food Point Located In The Centrum District, Where Meals Will Be Served, Meeting The Requirements Of The Swz (Description Of The Point Of Serving Hot Meals Above), As An Optional Element Of The Order Additionally Scored When Evaluating The Selection Of The Offer.
