The Subject Of The Order Is The Provision Of Funeral Services For The Burial Of Stillborn Children In The Śródmieście District Of The Capital City Of Warsaw, In Accordance With The Provisions Of: 1) The Act Of March 12, 2004 On Social Assistance; 2) The Act Of January 31, 1959 On Cemeteries And Burial Of The Dead, And; 3) Regulation Of The Minister Of Health Of December 27, 2007 On The Issuance Of Permits And Certificates For The Transport Of Human Remains; 4) Resolution Lx/1840/2009 Of The Council Of The Capital City Of Warsaw Of August 27, 2009 On The Principles Of Performing Funerals And Reimbursement Of Funeral Costs Carried Out By The Capital City Of Warsaw. Warsaw, As Amended Amended;5) Regulations Of The Cemetery Where The Burial Takes Place.2. The Detailed Scope Of Services Is Specified In The Description Of The Subject Of The Order (Opz), Which Is Appendix No. 1 To The Swz, And The Offer Form, Which Is Appendix No. 2 To The Swz.4. The Estimated Number Of Bodies To Be Buried During The Term Of The Contract Will Be 50, As Part Of A One-Time Burial In A Purchased Grave.5. A Funeral Should Respect The Dignity Of Persons And Their Families, In Accordance With The Norms And Customs Adopted In This Regard And In Accordance With The Provisions Set Out In The Act Of January 31, 1959 On Cemeteries And Burial Of The Dead, As Well As The Implementing Acts To This Act.
